Please be sure you print the Physical and Medical History Form and bring with you.
To get a physical your Medical History Form must be signed by a parent or Guardian.
Physicals are required each year for all incoming 7th thru 12 grade.
If you are unable to attend physcials on this date there are a couple of clinics in town that are offering physicals @ $25.

LBM Medical Clinic - 829 N Main Street,- Located inside the P&S Pharmacy, across from Brookshires Grocery.
Physicals prior to May 2024 will not be valid for the 2024-2025 School Year.
Please be sure you turn in your Physicals and Medical History to your coach or to one of the Athletic Trainers.
