END OF SEMESTER REPORTING REQUIREMENTS

    Congratulations on finishing another semester! End-of-the-semester reporting must be completed to maintain your Collins Scholarship. You must provide PDF documents for the information below by the following deadlines: Spring - June 1, and Fall - Jan. 1. If you do not provide all the requested information in accordance with the instructions by the deadline, you could lose your scholarship. 
 Title each with SEMESTER and YEAR, YOUR NAME, TYPE OF REPORT (Ex: Fall 2025 John Doe Grades)
    Check off each item as you gather your information. The information must be emailed to collinscholarship@cisd.org in this order:  

1. Grade report for the semester
2. Transcript
3. Financial statement of activity for the semester
4. Financial aid report for the semester
